Evaluation of Training During The Guided Perform Phase :

- Planned tool training has been completed, if any appropriate at this stage
- Draft Application Information Document (AID) sent to SME's for review (Completeness & Accuracy of the content of the respective sections in the document)
- OverallGuided Perform Skill Level
- Technical Infrastructure
- Mentees have application connectivity and security required to perform work
- Mentees have application access required to perform work (To be completed by SME/AM lead only)
Evaluation of work performed during the Assisted Perform Phase:
- Ability to perform assigned tasks (i.e. Enhancement/ Work Request,Production tickets,Adhoc user requests) or analysis scenarios with assistance from SMEs
- Mentee team has knowledge about the Problem Ticket & Change Management process for Production support
- Updated Application Information Document (AID) sent to SME's for review (Completeness & Accuracy of the content of the respective sections in the document)
- Draft Application Support Control Plan (ASCP) sent to SME's for review (Completeness & Accuracy of the content of the respective sections in the document)
- OverallAssisted Perform Skill Level (To be completed by SME/AM lead only)
Evaluation of work performed during the Parallel Perform Phase :
- Ability to perform assigned tasks (i.e. Enhancement/ Work Request,Production tickets,Adhoc user requests) or analysis scenarios with limited assistance from SMEs
- Final Application Information Document (AID) reviewed (Completeness & Accuracy of the content of the respective sections in the document) by SME's and baselined
- Final Application Support Control Plan (ASCP) reviewed (Completeness & Accuracy of the content of the respective sections in the document) by SME's and baselined
- Overall Parallel Perform Skill Level (To be completed by SME/AM lead only)
